We are currently recruiting for an experienced Specialist Support Coordinator to join a leading non-profit organisation. The Role

Reporting to the Manager, Support Coordination, your role will be to support participants with high complex needs and/or their family and carers in identifying and connecting with appropriate providers of supports. Your aim will be to reduce complexity, build capacity and choice and help in assisting connection to supports.
  
  
  
  • Assist participants in successfully coordinating their support needs in line with their personal goals and achieving outcomes of their NDIS plan
  • Assist with the implementation and facilitation of NDIS Plans while offering participants choice and control
  • Provide advice and support to other Support Coordinators in relation to delivery
  • Coordinators will work inclusively with participants, families, and carers to build great relationships and enhance participant supports
  • Assist participants to build capacity and skills to manage their support plans over time
  • Update and provide advice to participants, carers and families on the evolving NDIS environment
  • Build strong relationships with providers and other external stakeholders to ensure options and choice for participants
  • Develop action plans following consultation with participants, their families and other stakeholders to resolve challenges arising from complexity
  • Preparation of comprehensive reports together with a variety of other stakeholders
  • Promote the organisation in the area of Specialist Support Coordination
  
  
  
We are seeking a candidate with:
  
  • A tertiary qualification in a relevant subject such as Psychology, OT, Social Work or Health
  • Experience in the coordination of disability supports or case management
  • Working knowledge of the NDIS environment
  • Ability to read and understand support plans, or develop a support plan where it is missing;
  • Experience in the use of the MS Office suite, working knowledge of databases and demonstrated administration skills
  • Outstanding written and verbal communication skills
  • A high-level of organisational, time management and planning skills
  • strong problem solving skills
