The Role
Reporting to the Manager, Support Coordination, your role will be to support participants, their family and carers in identifying and connecting with appropriate providers of supports with a view to overcoming barriers, building capacity and developing skills.
- Assist participants in successfully coordinating their support needs in line with their personal goals and achieving outcomes of their NDIS plan
- Assist with the implementation and facilitation of NDIS Plans while offering participants choice and control
- Coordinators will work inclusively with participants, families, and carers to build great relationships and enhance participant supports
- Assist participants to build capacity and skills to manage their support plans over time
- Update and provide advice to participants, carers and families on the evolving NDIS environment
- Build strong relationships with providers and other external stakeholders to ensure options and choice for participants
- Prepare weekly reports, monitor expenditure and complete all associated administrative work
We are seeking a candidate with:
- Experience in the coordination of disability supports
- Working knowledge of the NDIS environment
- Ability to read and understand support plans, or develop a support plan where it is missing;
- Experience in the use of the MS Office suite, working knowledge of databases and demonstrated administration skills
- Outstanding written and verbal communication skills
- A high-level of organisational, time management and planning skills
